Session Information:

What to Expect on Your First Visit:

Allow an hour for the 1st session, all other sessions are 30 to 60 minutes in length unless otherwise agree.

Intake forms are emailed to you to be completed prior to the scheduled session.

Parent(s)/Guardian(s) needs to be available and present during session(s) in order to meet with minor(s) (consent must be signed by parent(s)/guardian(s) who have custody of the minor).

EAP Authorization, Co-Payment or Self-Pay due at the time of service.

EAP (Employee Assistance Program) programs cover full payment fee with no deductible or co-payment apply.

Pre-authorization and/or Co-Payment Cost (if applicable) from health insurance needs to be obtained prior to initial session to schedule an appointment.

Insurance provider questions to help determine your benefits:

Are mental health services covered by my health insurance provider?

Do I have to meet a deductible before fee is covered? If so, how much is the deductible and has the deductible been met?

Does my health insurance provider limit the number of sessions per calendar year? If so, what is the number of sessions covered?

Is a written referral needed from your primary care physician for services to be authorized and covered?

Cancellation Policy:

Office is open only for scheduled appointments, if you are unable to attend a scheduled appointment, at least 24 hours advanced notice is required. Since the time was unable to be use for another client, please be aware that you will be billed a cancellation fee, if it is not timely canceled, unless is due to an unexpected illness and/or emergency. Your support in helping keep time available to others will be appreciated.
